I've provided a doctors letter advising I work from home full time due to a chronic condition which is affecting me more due to a lot of stressors at work. My employer wants my doctor to also fill out an assessment form asking for more details. Am I obligated to get another form from my doctor?

Employers have an obligation to accommodate employees with disabilities to the point of undue hardship. However, employees must also participate and cooperate in the accommodation process, which may include providing your employer with supporting medical documentation. As you have already provided your employer with some supporting medical documentation, for us to assess whether your employer's request for further documentation is appropriate requires us to obtain more information from you about your circumstances. We strongly encourage you to contact us to set up a consultation with one of our employment lawyers who can fully review your circumstances, provide you with guidance and advice regarding your workplace rights, and assist you in navigating the situation in a manner that best suits your goals.
