From the information provided, it appears that the company is attempting to force you to go on disability benefits to avoid having to further accommodate you, and has referred this to its disability benefits provider. The disability benefits provider has now sent you a claim form asking for authorization to review your medical information to assess if you are eligible for the disability benefits being claimed.
